Admission decisions are made when the entire application process has been completed. The following information is required to complete an applicants’ file for evaluation:

1. Preliminary Information
   1. The completed “Application for Enrollment” (Click here for printable Application - PDF format: you will need Adobe PDF Reader to view and print this file). Click on the icon to get PDF Reader.
   2. Applicable testing fee:
      Pre-Kindergarten through Grade 3 ; $100 for Differential Abilities
      Scale (DAS) Testing ;
      Grades 4 through 12 ; $50.00 for Educational Records Bureau
      (ERB) Testing
   3. A recent photograph of the applicant (optional)
2. Previous School Records
   1. A current academic transcript or current report card (as applicable)
   2. Teacher recommendations, as required (Applicant should provide teacher(s) with a stamped envelope addressed to The Fayetteville Academy Admissions Office.)
3. Testing
   1. Arrangements for the appropriate, required testing should be made directly through the Admissions Office.
   2. Students applying to Pre-K-3rd Grade are required to schedule a classroom visit/observation. Students applying to Grades 4-12 are encouraged to visit classes in a grade level in which they are commensurate.
   3. Families are invited to schedule a tour of the campus.
